5. Auxiliary Nurse Midwife – 09 Posts
6. Lab Assistant – 01 Post
7. Nursing Orderly – 18 Posts
8. Dresser – 05 Posts
9. Linen Keeper – 01 Post
10. Dietician – 01 Post
11. Blood Bank Technician – 05 Posts
12. Lab Technician – 01 Post
13. E.C G Technician – 01 Post
14. Medical Record Technician (Jr.) – 03 Posts
15. Medical Record Officer – 01 Post

Age Limit: Candidate should have not exceeding 32 years for Medical Record Officer, E.C G Technician, Occupational Therapist, Dental Hygienist, Pharmacist, Auxiliary Nurse, Lab Assistant and Dietician posts, 37 years for Staff Nurse post, 18-27 years for Medical Record Technician (Jr), Dresser, Nursing Orderly posts and 18-32 years for Blood Bank Technician post as on 07-12-2012.

Educational Qualification: Candidate must possess Matric/ 10+2/ Degree/ Diploma/ PG in relevant disciplines.

Application Fee: Candidates need to pay challan of  Rs.125/- for Grade pay 1800-2400 and Rs.225/- for Grade pay 2401 and above for all other candidates (No fee for SC/ST/PWD/ Departmental/ Female Candidates & Ex Servicemen) payable at any SBI branch. For remittance of fee, candidates have to generate the prescribed challan from ESIC Website. The account number in which the amount of fee is to be deposited is available on challan.

Selection Process: Selection will be done on the basis of performance in interview.

How to Apply: Eligible candidates may apply through online from www.esic.nic.in website on or before 07-12-2012. After submission of online application, candidates take the printout of application duly signed by the candidate, send along with the required necessary documents in an envelope super scribed “Application for the post of ___ / Post Code No.__/ State__/ Online Registration Number__” should be sent by Speed Post or Registered post to the Medical Superintendent, ESIC PGIMSR and ESICH & ODC (EZ) JOKA, Diamond Harbour Road,Kolkata-700104 on or before 12-12-2012.

Instructions for Online Application:
1. Before Online Apply candidates should have a valid E-mail ID and enter this after reading instructions. A verification Code will be send to mail ID and enter this code to proceed.
2. Candidates need to fill application form properly, enter DD details like bank name, DD.No, in appropriate space provided in application.
3. Click on submit button after filling online application, A Unique User Name and Password will be generated and sent to your E mail Account. Note down this for further references.
4. Take a print out of System generated online application form and send along with attested photograph, DD in closed envelope must be reach to the above given address.
